Heart Transplant and Self-Change: WhatDying Teaches About Living

In a 10-year reflection on his heart attack and transplant, don Miguel Ruiz describes a near-death experience in intensive care where he felt his consciousness separate from his body—not as defeat but as liberation. When he survived with only 16% heart function, doctors advised he spend life on a sofa; instead, he chose to pursue his passion for teaching despite chronic pain. After eight and a half years living with progressive deterioration, he received a heart transplant in 2010. The core lesson: only by changing yourself—the protagonist of your own story—can you change the world around you, because your inner beliefs determine your outer reality.

What Happens When Consciousness Separates From the Body?

In the opening of his reflection, don Miguel describes the moment of his heart attack with remarkable clarity. Rather than depicting it as pure crisis, he frames his experience as one of release. "I conquer death," he explains, "by letting go of my physical body. That would make me feel so excited, because my body will feel when I live, and that will be the last thing my body will remember me—life."

This was not a metaphorical statement but a lived experience. During the acute event, don Miguel recalls sharing this moment with his son Jose while in intensive care. His son asked if he understood what had happened, and don Miguel confirmed: "Yes, father. Now you can go up because there are many other people waiting for me." What emerges from this account is not despair but a sense of permission—as though the physical crisis opened a window onto a reality beyond the body's suffering.

Don Miguel notes that he wished to share this experience with everyone, but the medical constraints of intensive care made that impossible. Nevertheless, the private moment became the foundation of his subsequent choices. He had glimpsed, directly and undeniably, that his identity was not synonymous with his body, and that awareness would define how he faced the years ahead.

How Did He Respond to a 16% Heart Capacity Diagnosis?

When don Miguel emerged from a nine-week coma, the prognosis was stark. Doctors told him he was living on only 16% of his heart's normal capacity. Their recommendation was clear: spend the rest of his life at home, confined to a sofa, "watching TV or reading books." This was not cruel—it was medically prudent. A person in that condition would be expected to live one, perhaps two years.

Don Miguel's response was unequivocal: "There is no way that I will stay at home waiting for death. No, I will go out and I will look for death by doing what I love to do with my life, by following my passion." For over 20 years, his passion had been teaching a specific message: "Everything is possible if this is what you really want to do. The only thing you need is a strong will and discipline, and with that, you can make your dreams come true."

This was not denial or recklessness. Rather, it was a conscious choice about how to spend whatever time remained. The medical verdict assumed that longevity without vitality was the default goal. Don Miguel inverted that logic: he would pursue vitality even if it shortened his time. He would live fully, or not at all.

What Does It Mean to Live With Chronic Pain But Not Suffer?

For eight and a half years, don Miguel lived in constant, progressive pain. His body deteriorated day by day. Yet he observed something he found crucial to name: pain and suffering are not identical. "My body was always in pain, but it was not a justification to suffer," he says. "Because I really enjoy life, and hardly anybody noticed that my body was in pain, because I kept that wonderful smile, completely sincere, and I really enjoyed everything that I could perceive."

This distinction is not semantic. Pain is a physical sensation. Suffering is the meaning and resistance we add to that sensation. Don Miguel chose to respond to his pain not with complaint or contraction, but with enjoyment. He enjoyed the activity when he was active, and he enjoyed the rest when he was at rest. This was not spiritual bypassing or forced positivity. Rather, it was a deliberate allocation of his attention: his body was in pain, but his mind and spirit remained engaged with life.

This capacity became his actual work during those eight and a half years. He did not advertise his deterioration. He continued to teach. By September 2009, he made the decision to pursue a heart transplant, applying for the procedure knowing that if he did not receive one, he would likely not survive 2010. The waiting period—a full year of medical testing—was itself an exercise in this principle: endure the limitation, but do not let the limitation define the scope of your consciousness.

How Did the Heart Transplant Change His Understanding of Life?

The call came at midnight on October 9, 2010. Don Miguel was in Texas giving a lecture with his son when hospital told him a donor heart was available. He had three hours to reach Los Angeles. His response to his older son was instructive: he refused to let his son abandon his work to accompany him. "I told him: 'You have work to do. This is not an excuse for you to stop doing what you are doing and enjoying what you are doing.'"

Then he articulated his frame of mind going into surgery: "For me, it's a win-win situation. I have nothing to lose. I'm not afraid at all of this surgery. Let's say I don't wake up from the surgery—it's crazy, because I will not have that pain any longer. And if I wake up from the surgery, that means the whole process was a success, and then I would experience what it means to live with somebody else's heart. Which is a great experience. Can you imagine living with somebody else's heart?"

This statement reveals the spiritual coherence of his approach. He had already accepted death—had already experienced its dissolution. The transplant was not a rescue from an intolerable situation but an opportunity to explore a new dimension of existence: what does it mean to live in a body that is not biologically "yours" but given to you by another? What does gratitude toward the unknown donor become in daily life?

The surgery succeeded. Don Miguel's recovery, as he describes it, was a "miracle." He experienced no rejection symptoms. His strength returned steadily. By September 2011, doctors cleared him to resume his full work. Since then, he has traveled extensively: three times to Canada, multiple visits to Mexico, Ecuador, Japan, Costa Rica, Colombia. His 2012 calendar was completely full.

What Is the Core Spiritual Teaching He Distilled From This Decade?

On the 10-year anniversary of his heart attack, don Miguel returns to a single, fundamental proposition: "Please help us change the world. Not the planet Earth, not humanity—the world that you create in your own head. Because only you can change it. Only you know what you like and what you don't like. Only you know what makes you happy and what makes you unhappy."

This is not a retreat into solipsism. Instead, he argues that the world that exists for you is the world you have authored through belief. "The key to change your world is to change the main character of your story—which means yourself. If you change anybody else, it didn't make a change in your world. It didn't make a difference. But if you change the main character—which means what you believe you are—just like magic, everything around you starts changing."

The benefit of changing yourself does not stop with you. It ripples outward: "The benefit after that is your beloved, the one that you live with, your children, your parents, your brothers, your sisters, your family, your friends, your community, your country—and finally, the entire humanity."

This teaching is consistent with the lived example he offered. He did not ask his doctors to change their prognosis. He did not ask his sons to alter their lives. He did not ask the world to become more accommodating to his pain. Instead, he changed his belief about what was possible—that a person with 16% heart function could live fully, travel the world, continue teaching, and find joy in every moment. That internal shift was not magical in the fantasy sense; it was magical in the sense that it reorganized his actual circumstances, drawing from the world the resources and opportunities that matched his new internal stance.

Where to go from here

If you are drawn to this teaching, consider several directions. First, examine your own relationship to limitation. Don Miguel's example was not that limitation doesn't exist—his pain was real—but that limitation need not colonize your entire internal world. Where are you treating a constraint in your circumstances as a sentence on your consciousness? Second, explore the idea that you are not your body, by contemplating what part of your identity actually depends on physical conditions. What would remain if your health changed? What would you still choose to do? Third, notice where you are waiting for external change before allowing yourself to live. Don Miguel chose to live fully despite medical advice to the contrary. What would you need to believe about yourself to make a similar choice? Finally, attend to your influence on others—not through trying to convince them, but through the quiet radiation of your own authentic engagement with life. Don Miguel's recovery baffled his doctors in part because he exceeded every behavioral pattern his illness should have produced. That exceeding itself became teaching.

[0:01] now as you can see

[0:04] we conquer death

[0:08] then leaving my physical body it doesn't

[0:11] mean that i was defeated by death

[0:13] no the opposite i

[0:16] let go of my physical body

[0:19] and that would make me feel so excited

[0:23] because my body will feel

[0:26] when i live

[0:30] and that will be the last thing that my

[0:33] body will

[0:34] remember me

[0:37] life

[0:41] and i tell not do you understand he said

[0:44] yes father

[0:45] he said now you can go up because

[0:47] there's many other people waiting for me

[0:51] and he left

[0:55] but for me this moment was extremely

[0:58] important

[1:00] because i couldn't share that experience

[1:03] with all the people that i wish number

[1:06] one because it was impossible that they

[1:07] come to see me in intensive care

[1:11] and that the doctors allowed them to

[1:12] come

[1:15] but in my imagination when i have the

[1:17] heart attack in the beginning

[1:19] i imagine myself telling the whole world

[1:21] and everybody

[1:23] what i was sharing with my son jose

[1:27] when my son michael jr came

[1:30] i'm not really myself anymore

[1:33] i barely remember when he came to see me

[1:37] because he come to see me i didn't know

[1:41] i had a heart attack

[1:43] i was already dreaming and i thought

[1:46] that he was in his way

[1:49] to baja california south

[1:52] to see his girlfriend

[1:55] and i thought he just stopped just to

[1:57] see me and say

[1:59] father i love you i gonna see my

[2:01] girlfriend and i see you later

[2:03] i give it all my blessings

[2:06] and after that i went into coma for nine

[2:09] weeks

[2:12] nine weeks in coma

[2:16] when i came out of the coma the doctors

[2:20] let me know that i was living only with

[2:23] the sixteen percent of my heart capacity

[2:28] and what they recommend is that i spent

[2:32] the rest of my life

[2:33] in a sofa watching tv

[2:37] or reading books

[2:41] when i was at home and i talked to my

[2:43] children

[2:45] i told them there is no way that i will

[2:47] stay at home

[2:49] waiting for death no i will go out

[2:53] and i will look for death by doing what

[2:56] i love to

[2:57] do with my life

[3:00] to follow my passion which is

[3:04] to share the message that i've been

[3:06] sharing for

[3:07] more than 20 something years

[3:10] to let everybody know that everything is

[3:13] possible

[3:14] if this is what they really want to do

[3:17] that the only

[3:18] thing that they need is a strong will

[3:22] and discipline and with that

[3:26] they can make their dreams come true

[3:33] the doctors told me that i

[3:36] may survive one maybe two years

[3:42] then i start planning everything for

[3:47] my future debt whenever it happens

[3:49] doesn't matter

[3:53] but the only problem that is that i was

[3:56] living in extreme pain

[3:58] all the time my body was always

[4:02] in pain but it was not a justification

[4:06] to suffer because i really enjoy

[4:10] life and hardly anybody

[4:13] noticed that my body was in pain because

[4:16] i

[4:16] keep that wonderful smile completely

[4:19] sincere

[4:20] and i really enjoy everything that i can

[4:22] perceive

[4:25] i enjoy the activity as i enjoyed the

[4:29] rest

[4:34] and for eight and a half years i was

[4:36] living in that condition

[4:39] and every day it was getting a little

[4:41] worse and a little worse

[4:43] than but by 19 2009

[4:47] in august i decided that

[4:51] because i didn't die this two years

[4:55] i will try to have a heart transplant

[4:58] i apply it took a whole year

[5:02] for the doctor to test all my physical

[5:04] body

[5:05] in order to allowed

[5:09] to have the heart transplanted they put

[5:10] mendelius in august in 2010

[5:14] and then they told me well uh

[5:17] you may have the heartland spot between

[5:19] six months

[5:20] and one year but i knew that it would be

[5:24] sooner

[5:25] i was thinking maybe by november i may

[5:28] have my new heart

[5:31] and i tell that to myself because i was

[5:33] really feeling that

[5:34] i may not survive the end of

[5:39] the year 2010

[5:43] then in october 9

[5:46] 2010 they called me because i was

[5:50] doing a lecture in texas with my santon

[5:54] michael rich junior

[5:57] they call me at midnight and they say we

[6:00] have a heart for you

[6:02] you have to be here in the hospital in

[6:05] around three hours

[6:07] from texas to l.a in three hours

[6:12] i say i will be there

[6:15] well it took me six hours to arrive

[6:20] to los angeles

[6:24] and get to the hospital

[6:27] where the heart was waiting for me

[6:30] before i leave texas i tell my older son

[6:33] because in that moment say

[6:35] okay i will go with you i say no

[6:38] you have a work to do you have to finish

[6:41] your work

[6:43] this is not an excuse for you to to stop

[6:46] doing what you are doing to enjoy what

[6:47] you're doing

[6:50] because you have to understand that for

[6:51] me it's a win-win situation

[6:54] i have nothing to lose i'm

[6:58] not afraid at all to that surgery

[7:02] let's see that i don't wake up from the

[7:04] surgery

[7:05] it's crazy because i will not have that

[7:07] pain any longer

[7:10] and if i wake up from the surgery that

[7:12] means that

[7:13] the whole process procedure was

[7:17] a success and then i would experience

[7:21] what it means

[7:22] to live with somebody else's heart

[7:26] which is a great experience

[7:29] can you imagine live with somebody

[7:31] else's heart

[7:34] to come back to life

[7:38] and as you can see that you can enjoy

[7:39] even more

[7:41] every single moment of your life

[7:46] as you can see the surgery was a success

[7:53] and my recovery has been we can call it

[7:56] a miracle

[7:59] i never have not a little a little

[8:02] symptom

[8:03] of rejection and i get stronger and

[8:07] stronger

[8:08] than in september last year

[8:12] they give me the green light that i can

[8:15] go back and do

[8:16] my work and since september until now

[8:22] i really went three times to canada like

[8:25] another two three times to mexico to

[8:28] different states in united states

[8:31] i went to japan i went to costa rica

[8:35] i went to colombia i went back to mexico

[8:40] once again and the whole calendar

[8:44] 2012 is completely full

[8:50] today i celebrate

[8:56] my death day 10

[8:59] years of the heart attack

[9:03] and once again i ask

[9:06] all of you that please

[9:10] help us to change the world

[9:14] not the planet earth not humanity

[9:19] the world that you create in your own

[9:22] head

[9:24] because only you can change it because

[9:27] only you know what

[9:28] is what you like and what is what you

[9:30] don't like

[9:32] only you don't what make you happy and

[9:35] what make them happy

[9:37] then the key to change your world is to

[9:41] change the main character of your story

[9:44] which means yourself if you change any

[9:48] anybody else it didn't make a change in

[9:51] your world

[9:52] it didn't make a difference then

[9:55] if you change the main character

[9:59] which means what you believe you are

[10:01] just like magic

[10:03] everything around you start changing

[10:07] and the benefit after you're on

[10:11] is your beloved the one that you live

[10:15] with

[10:16] your children your parents your brothers

[10:20] your sisters

[10:24] your family your friends

[10:28] your community your country

[10:32] and finally the entire humanity

[10:38] then please help us to change the world

[10:42] by changing your own

[10:47] with all my love to all of you

Yes, according to don Miguel's example. He distinguishes between pain (a physical sensation) and suffering (the mental narrative around it). For eight and a half years with only 16% heart capacity and progressive pain, he maintained genuine enjoyment of life by directing his attention toward what he could perceive and do, rather than amplifying his limitation. The key is that vitality is not the opposite of illness; it is a choice of how you relate to your circumstances.
Don Miguel's approach was not to deny the medical reality—he took seriously that he had one to two years—but to refuse to treat the prognosis as a directive for how to spend that time. Instead of retreating to a sofa, he chose to pursue his passion. He also pursued the medical option of transplant when available. The deeper lesson: accept the diagnosis; reject the implicit limitation it places on your agency and meaning-making.
In don Miguel's teaching, your internal belief about what is possible becomes the filter through which you perceive and act in your circumstances. When he changed his belief from 'I am a dying person who should rest' to 'I am a person who will live fully and pursue my passion,' his choices, energy, and what opportunities came toward him all shifted. He traveled, taught, and received a transplant. This was not visualization magic but a real reorganization of his agency.
In don Miguel's account, his near-death experience in the heart attack and coma gave him a direct felt sense that his consciousness could separate from his body. Rather than being frightened by this, he found it liberating—a proof that he was not merely a body, and therefore that death of the body was not the annihilation of self. This shifted his relationship to fear for the remaining years.
No, according to don Miguel. He argues that changing yourself is the only leverage point that actually creates world change. If you change others or external circumstances without changing your own beliefs, you only create friction and no lasting shift in your experience. But when you transform your internal world—your belief about what is possible—that radiates outward to your family, community, and eventually all humanity, because they respond to the different energy and choices you embody.
Don Miguel's method was to actively enjoy what he could perceive in each moment: the activity when active, the rest when resting. He was not denying pain but refusing to let pain monopolize his attention and emotional range. This required conscious will and discipline, which he had developed over decades of spiritual practice. It was not automatic or easy, but it was possible.
Don Miguel frames the transplant as a profound mystery: he is now literally living in a body animated by another person's organ. This daily, physical reminder that his life depends on another's gift—from a donor he will never meet—becomes a teaching in humility and gratitude. He does not take the gift as his right but as an opportunity to experience what it means to live in radical dependence on others.
